Vaccine Certificate PDF (INCLUDES ALL YOUR PET’S VACCINES THAT ARE CURRENT)

Description

• A vaccine certificate includes all vaccines that are current, including rabies.
• If no vaccine record is found in our database, or if the all the vaccines we locate have expired, a certificate cannot be provided unless a copy of a current record from Low Cost Pet Vaccines is provided to us.  Occasionally our system omits records and we are happy to add current records to our pet parent’s files if we are provided with one of our receipts.
• IF you have been asked to provide a ‘rabies certificate’,  request “rabies certificate” from our products.
• If a vaccine has expired, it will not show up on the certificate.
• If you would like a record of all vaccines purchased from our clinic, even if they have expired, please select ‘vaccine records’.
• IF your veterinarian is requesting ‘all records including medical notes’, please select ‘medical records’
